A beneficiary is the person or entity you name in a life insurance policy to receive the death benefit. You can name: One person.
(a) & (b): CGHS card is valid in any CGHS Wellness Centre in CGHS covered cities all over India and CGHS Wellness Centres (WCs) cannot refuse medical facilities to the beneficiaries not enrolled with the WC.
Old CGHS card and a Death Certificate need to be attached with the application. A serving employee on marriage or on the birth of his/ her child may get the names of spouse /child added to the card after submitting the form for addition duly endorsed by his department.

In Pregnancy Cases. Once the pregnancy is diagnosed/confirmed by the Government/CGHS doctors including the Medical Officer working at the dispensary level, the Head of the Department of concerned Department/Ministry may permit admission for confinement purpose in the private hospitals recognized under CGHS.
Hospitalization in emergencyIn emergency CGHS beneficiary may be admitted to any Govt/private hospital, but the reimbursement will be as per CGHS rates.
