The sound signal from your Tv, Apple TV or whatever streaming device you use and want amplified will connect to the “in” HDMI port. The purpose of the HDMI “out” is usually to go out to the TV for audio and video connected to the HDMI in on the TV, usually the (ARC) HDMI port for that application.

As earlier mentioned, a Smart TV has pretty much all of the functionalities of a media streaming device; so ideally, you do not need an extra media streaming device when you have a smart TV.
