To stop Adella from killing Arianna, simply remove Arianna's Blood Vial from your inventory and instead take Adella's Blood. If you defeat Rom with Adella's Blood in your inventory, Adella will not kill Arianna.
While Iosefka is not replaced by her imposter, she will give any Iosefka's Blood Vials freely, so long as they do not have one already. If players attack her door too many times, she will refuse to speak with them. Drops a Iosefka's Blood Vial when found after being "replaced" by Imposter Iosefka.
To get the cord, you'll have to kill her baby.
The third Umbilical Cord is obtained by going to the Healing Church Workshop and dropping down by the bit where the path breaks on the lower section. You should now be in a circular room.You need the item Cainhurst Summons found in 1st Floor Sickroom to enter the Forsaken Cainhurst Castle. To get to the sick room, go to the forbidden woods lamp. Run across the bridge and follow the path. Stick to the left wall.
The steps you must take to unlock Cainhurst Castle are:
- Defeat Vicar Amelia and watch the cutscene after touching the skull.
- Fight through the Forbidden Woods and reach the back entrance to Iosefka's Clinic.
- Collect the Cainhurst Summons from inside Iosefka's Clinic.
- Defeat the Witch of Hemwick at Hemwick Charnel Lane.
Iosefka's Blood Vial. This refined blood, highly invigorating, restores a larger amount of HP. The product of a slow and careful refinement process, this rare blood vial appears to be a clinic original. Iosefka's Blood Vial is a Consumable Item in Bloodborne.
After beating Rom, the Vacuous Spider, you probably saw some monsters in the area. If you wish to kill them and collect the items, take the lamp to Hunter's Dream and then access the Grand Cathedral Lamp. If you prefer to continue pushing forward, scroll down to our section for Yahar'gul, Unseen Village.
If you "kill" Gehrman enough within Hunter's Dream he will vanish into mist and becomes unavailable until you defeated Mergo's Wet Nurse. He also disappears at a specific point in the story. However if you attacked Gehrman at the foot of the Great Tree, he immediately becomes hostile.
When the player accepts Gehrman's offer, the death is a sign of the player waking up from his or her dream. However, which one is dream and which one is reality is still debatable: The Hunter's Dream is no more than a dream, Gehrman is indeed freeing you from the dream, and the player awakes at reality.
Third Umbilical Cord Usage
Can be used to access the third ending in the game, consuming 3 Cords before the fight against Gehrman, The First Hunter or at least before he dies, and unlock the fight against Moon Presence. Increases your Insight by 3 when consumed.The only way you can save her is not to send her anywhere. If you send her to the chapel she will either give birth and die, give you too much of her whores blood and the nun will kill her or if you invite the cannibal then he'll kill everyone.

If you are killed by the boss you will respawn in the Hunter's Dream. Upon its defeat a cutscene will initiate and the game will end; you will then obtain the "Childhood's Beginning" trophy.Arianna's Baby Umbilical Cord
When you meet Arianna, you must send her to the Oedon Chapel. After you've killed Micolash, Host Of The Nightmare, she's going to give birth to a baby below the starting room of Cathedral Ward. When you finish the boss fight, awaken at the Cathedral Ward lamp.When should you consume the umbilical cords? You can consume them in any order at any time in the game pre-killing Gehrman. If you wait, you can get all three ending-trophies on one playthrough---if that's your thing. Just before final fight, upload save to PS+.
Rom has minor weaknesses against fire. Applying Bolt Paper or Fire Paper to a weapon will help deal extra damage. The Tonitrus is very effective, as Rom is a Kin Great One and Kin will take extra damage from Bolt. The most common strategy is to ignore the spiders completely and use hit-and-run tactics.
Killing NPCs is not recommended. If you do kill them, they usually drop a badge. Badges add messengers to the bird bath in the Hunter's Dream. And these extra messengers add more items to the shop.
If you decide to fight Imposter Iosefka, she uses the following weapons/tools to try and kill you:
- A Threaded Cane with Slow Poisoning.
- A Repeating Pistol.
- She usually throws a Numbing Mist at the start of the fight.
- She frequently makes use of Augur of Ebrietas.

- Speak to Gherman in Hunter's Dream.
- Visit the Cathedral Ward and look for a new door on the right, from the direction you face as you warp into the zone.
- Take the lift to the top.
- Immediately after entering, step off the middle beam on the left hand side.
