Subsequent to computerization no separate permission is required and CGHS card is valid for availing facilities from any CGHS Wellness Centre in India. For any assistance, Additional Director of the city (Additional Director CGHS HQ in case of Delhi) may be contacted.
In case the identity card is lost, the beneficiary should lodge a complaint with the police station and also report the matter to the CMO In-charge of the dispensary, the sponsoring Department/ Ministry, and to the Additional Director, CGHS in the city.
The procedure for doing so is as follows: (1) Through CMO -In Charge's Module- Beneficiary can visit CMO-In Charge Of his her CGHS Wellness Centre with AADHAR Card to get his her and family members AADHAR Number linked to CGHS Beneficiary Number. c. Sign in with Ben ID and password if you are already registered.
In case of Pensioners:
- A printout of the duly filled up online Application Form by visiting the website cghs.nic.in.
- Copy of the PPO.
- Copy of the LPC (Last Pay Certificate).
- Proof of residence.
- Documents for proof of age for all applicants.
- Legal documents for widowed or divorced daughter.
- Aadhar card copy.

Documents required for Deletion of Family MemberDeletion form forwarded by Sponsoring Authority with seal & signature. In case of Death, A copy of the Death certificate has to be given. Original CGHS Plastics card of person to be deleted to be surrendered. CGHS card copy of the Principal Card holder to be given.
The pensioners / family pensioners have to get their names registered with any of the CGHS dispensaries after submitting an application in the prescribed proforma for the issue of a CGHS identity card. MEDICAL ALLOWANCE FOR RETIREES RESIDING IN AREAS NOT COVERED UNDER CGHS.
The claim is to be submitted to the concerned department by serving employees and to the CMO I/C of the CGHS wellness Centre (where the CGHS card is registered) by the pensioner beneficiary within 3 months of discharge from the hospital. The claim is to be submitted in duplicate in the prescribed form.
CGHS is a special health scheme for the government employees. The medical facilities are provided through Wellness Centres (previously referred to as CGHS Dispensaries) / polyclinics under Allopathic, Ayurveda, Yoga, Unani, Sidha and Homeopathic systems of medicines.
If a pensioner having lifetime CGHS card issued at one place, shifts to another place then the card is to be surrendered in the office of AD of the first place & after obtaining surrender certificate and NOC, a fresh card shall be obtained from AD of the other place.
BENEFICIARY ID = 8 digit number that identifies your individual demat account number with your demat service provider. Combination of 16 digits = DP ID + BENEFICIARY ID = your unique demat ID. You Unique Beneficiary ID remains the same, as long as you maintain your demat account with the same Demat service provider.
Health benefits card will be provided by government and save deposited of the same is the responsibility of the employee, To add the names of family members in CGHS Plastic card, employee should fill their details filled in application form with attached photo, where the name of family member to be added must be as per
Procedure to get a CGHS Card. Eligible serving employees/pensioners have to apply online or in the prescribed form. The form should be completely filled up with individual photographs pasted as specified in the form.
The steps involved in printing of the card are given below:
- Visit CGHS portal cghs.nic.in.
- Click 'Beneficiary Login'
- Enter Beneficiary ID and Password, and sign in.
- Click 'Print Card' for the beneficiary whose card needs to be printed.
- Enter the One Time Password (OTP) sent on registered mobile.
- Click 'Print CGHS Card'
You can update your Address online in Self Service Update Portal (SSUP). For other details updates such as Demographic details (Name, Address, DoB, Gender, Mobile Number, Email) as well as Biometrics (Finger Prints, Iris & Photograph) in Aadhaar you will have to visit Permanent Enrolment Center.
How to Add Your Mobile Number to Aadhaar Card
- Visit your nearest Aadhaar Enrolment or Update Centre.
- Fill out the Aadhaar Correction Form.
- Fill in your current mobile number that you want to update in your Aadhaar.
- Submit the form and help them with your biometrics for authentication.
Serving employees on transfer from one CGHS city to another CGHS city shall submit an application to the Additional Director of the CGHS City (forwarded by the Ministry/ Department from where he /she is being transferred) along with copy of the transfer/ relieving order for transfer of CGHS Cards to another CGHS city
