Answer is Kirito was never a girl in GGO. While he looked very feminine this time around he was still classified as male. Since Kirito was trapped in SAO for 2 years and converted his account to GGO this explains why he got such a rare avatar. Kirito is not the only one who got a rare avatar model in GGO.
